Hiring: Epic Healthy Planet Analyst – 100% Remote

Location: Remote

Contract: 13 Weeks with possible extension

Schedule: Monday–Friday | 8 hours/day | 40 hours/week

Pay: $90/hr on W2

Position OverviewWe are seeking an experienced Epic Healthy Planet Analyst to support Population Health initiatives, care management workflows, quality programs, and value-based care initiatives.

Epic Healthy Planet certification is required.

Key Requirements

• Epic Healthy Planet certification – Required
• Experience with Epic Healthy Planet build, configuration, and optimization
• Experience with Compass Rose / advanced risk stratification tools preferred
• Experience integrating external data sources such as claims, SDOH, and HIE feeds
• Strong understanding of HEDIS and eCQM quality measure logic
• Experience with population health, care management, and utilization workflows
• Experience with registries, care gaps, risk scoring, patient attribution, and reporting
• Ability to troubleshoot data integrity and reporting issues
• Experience working with Epic Clarity/Caboodle and downstream reporting platforms preferredResponsibilities
• Configure, optimize, and support Epic Healthy Planet build
• Develop and maintain population health registries, dashboards, reports, and care gap logic
• Build and support care plans, protocols, assessments, and outreach workflows
• Analyze population health workflows and translate business needs into system enhancements
• Support HEDIS, Medicare Stars, and other quality/value-based care initiatives
• Validate data flows between Epic, Clarity/Caboodle, and reporting platforms
• Troubleshoot registry, attribution, reporting, and data-quality issues
• Collaborate with clinical, Population Health, IT, analytics, and operational teams
• Support testing, validation, upgrades, and change management
• Provide end-user support and issue resolution
• Maintain documentation for Healthy Planet build, workflows, and reporting logic
