JustSoftware builds HIPAA-compliant software for providers, clinics and care teams. That includes EHR/EMR systems, patient portals, telemedicine, medical billing, and HL7, FHIR and EHR integrations. Clients own their code and data, a BAA is signed before any PHI is touched, and we follow SOC 2-aligned practices on every project.

About the role

You’ll join our healthcare engineering team as a full-stack engineer, building and supporting platforms used by clinicians and staff every day. You’ll work across client projects, own features and integrations end to end, and extend our coverage into US business hours.

What you’ll do

* Build EHR/EMR, patient-portal, billing and care-coordination features end to end

* Build and maintain integrations: HL7 v2 interfaces, FHIR R4 APIs, payment providers, SMS and email services, vendor APIs and webhooks, and SFTP file exchanges

* Design secure data models, with role-based and record-level access control and audit logs

* Migrate data from legacy systems (CSV, PDF and database exports) with full data integrity

* Turn clinical and operational workflows into clear, reliable software

* Provide production support during your shift and join an on-call rotation: monitoring, incident response, fixes and security patches

* Write well-tested code, ship through CI/CD, and keep documentation and runbooks current

* Take part in code reviews and, when needed, client discovery calls

Must have

* 3+ years building production software, including HIPAA-compliant work with PHI, access control and audit logs

* React and TypeScript (Vite or Next.js)

* Node.js with NestJS (or Express), REST APIs and webhooks

* PostgreSQL, with Drizzle, Prisma or a similar ORM

* Hands-on HL7 v2 and FHIR R4 integration experience

* AWS (ECS, RDS, S3, Cognito) and Docker

* Understanding of clinical and practice workflows: scheduling, charting, orders, results and claims

* Testing (Vitest or Jest, Playwright) and CI/CD (GitHub Actions)

* Experience supporting live production systems: debugging from logs and metrics, handling incidents calmly

* Able to work US business hours and join an on-call rotation

Strong plus

* Integration with Epic, Oracle Health (Cerner) or athenahealth, including SMART on FHIR

* Medical billing and X12 EDI (837, 835, 270/271), clearinghouses

* Telehealth or patient portals

* PDF generation and e-signature flows

* Healthcare AI: ambient scribes, LLM workflows with clinician review

* Mobile development (Flutter or Swift), or Python
