Description

We are looking for a Senior Java Engineer to work on a US payer-side healthcare platform focused on FHIR-based API development, system integration, and healthcare data processing. The role involves developing, enhancing, and maintaining FHIR APIs using Java, Spring Boot, and HAPI FHIR, along with building structured and unstructured data extraction pipelines using Spring Batch. The engineer will collaborate with cross-functional teams to improve API performance, ensure reliable data processing, integrate services through Apache Kafka, and deliver scalable backend solutions. The role also requires strong debugging, testing, and problem-solving skills, along with the ability to work with an existing codebase and communicate progress clearly with US-based stakeholders.

Responsibilities

Roles and Responsibilities

Develop, enhance, and maintain healthcare APIs using Java 17+, Spring Boot 3, and HAPI FHIR.
Build and integrate FHIR R4 resources, Bundles, references, extensions, and profiles to support healthcare data exchange.
Work on FHIR-based prior authorization workflows and related healthcare interoperability requirements.
Design, develop, and maintain batch processing pipelines using Spring Batch for structured and unstructured data extraction.
Implement batch jobs with appropriate processing, error handling, retry, restart, scheduling, and monitoring mechanisms.
Extract information from documents such as PDFs, Word files, HTML pages, and scanned images, and map the extracted data to appropriate FHIR resources.
Develop and maintain Kafka producers and consumers for reliable asynchronous communication between the FHIR platform and payer portal.
Design and optimize SQL queries, database transactions, and data processing operations across relational databases, preferably Microsoft SQL Server.
Implement REST integrations with appropriate timeout, retry, error-handling, and message-correlation mechanisms.
Troubleshoot application issues, analyze logs and stack traces, and resolve performance, integration, and data consistency problems.
Write unit and integration tests using JUnit 5, Mockito, and relevant Spring Boot testing tools.
Participate in code reviews, Git-based development workflows, API testing, and Agile ceremonies.
Collaborate with engineering teams and US-based stakeholders, providing clear progress updates and taking ownership of assigned deliverables.
Follow engineering best practices for application security, reliability, maintainability, and performance. Requirements Technical Skills
Strong hands-on experience in Java 17 or later, including modern Java features, collections, streams, generics, Optional, exception handling, and object-oriented programming principles.
Strong experience with Spring Boot 3, including REST APIs, dependency injection, configuration, profiles, conditional beans, transaction management, and application monitoring.
Hands-on experience with Spring Batch, including jobs, steps, ItemReader, ItemProcessor, ItemWriter, chunk-oriented processing, retry, skip, restart, and job configuration.
Practical experience with FHIR R4 and healthcare interoperability, including working with FHIR resources, Bundles, references, and FHIR APIs using the HAPI FHIR library or equivalent hands-on FHIR platform integration experience.
Strong SQL skills, preferably with Microsoft SQL Server, including joins, CTEs, window functions, subqueries, indexing, query optimization, and transaction management.
Experience with JPA/Hibernate, relational database design, and working with multiple data sources.
Hands-on experience with Apache Kafka and Spring Kafka, including producers, consumers, message acknowledgements, error handling, and reliable message processing.
Experience integrating REST services and implementing appropriate timeout, retry, and asynchronous processing mechanisms.
Knowledge of document processing and data extraction from formats such as PDF, Word, HTML, and scanned documents.
Experience with JUnit 5, Mockito, unit testing, integration testing, and API testing using Postman, Bruno, or similar tools.
Strong debugging and troubleshooting skills, including analyzing application logs, stack traces, and unfamiliar codebases.
Familiarity with Git, pull requests, code reviews, and Agile software development practices. Good to Have
Experience with HAPI FHIR JPA Server internals, custom operations, interceptors, resource DAO interfaces, and FHIR validation.
Knowledge of HL7 FHIR implementation guides, particularly Da Vinci CRD, DTR, and PAS.
Understanding of healthcare payer workflows, prior authorization, claims, utilization management, or X12 278.
Experience with PDFBox, Apache Tika, OCR tools such as Tesseract, or AWS Textract.
Familiarity with SQL on FHIR, FHIR Bulk Data export, NDJSON, SQL Server JSON functions, or flattening nested FHIR resources into relational tables.
Exposure to AWS services such as ECS and S3, Docker, and CI/CD pipelines.
Familiarity with application observability tools such as Micrometer, Prometheus, Grafana, or Datadog.
Experience with Testcontainers for database and Kafka integration testing.
Exposure to cloud-based healthcare platforms and large-scale backend systems. Preferred Candidate Profile
Strong backend engineering fundamentals with practical Java and Spring Boot experience.
Hands-on exposure to FHIR APIs or healthcare interoperability platforms.
Ability to work independently, take ownership of technical tasks, and deliver solutions in an existing codebase.
Strong analytical, debugging, and problem-solving abilities.
Good communication skills and the ability to provide clear technical updates to US-based stakeholders.
